Yes, my “history related” building/company/organization plans to participate in the 2011-2012 Back Through Time
History Trail. Enclosed is the $65.00 marketing fee to pay for a ¼th page ad in the official trail brochure which will be
available to the public in May. The Trail is now a year round rural marketing initiative which covers the period from May
2011 through April 2012. Please provide information below:

Note: County Historical Commissions, Chambers of Commerce: It is not necessary for you to complete this form;
instead please send a narrative about your county/city with listing of major events, etc. Send narrative, photos, and
